Andrew Chan
Andrew Chan works at St Vincent’s Hospital in Melbourne and manages the Assessment Liaison and Early Referral Team that works out of the Emergency Department supporting homeless clients. In this podcast we discuss with Andrew how the program has changed over the last 20 years in response to the changing profile of clients coming to the hospital and the important role the team has in supporting and diverting clients from hospital care and linking them to community programs.
